The Gateway design and tools target recognized failures in today’s care delivery and transitions of care: poor education, low health literacy, services fragmentation and social determinants of health. The web and call center model build on evidence that the Re-Engineered Discharge project emphasizes by putting tools and resources in the hands of patients and families in addition to clinicians. The design builds upon the findings of the Remington Report supporting community-based care transitions models currently being funded by the Centers for Medicare and Medicaid Services.
The Gateway combines proven tools with patient and family oriented navigation and explanation.
