Connecting Clinicians, Community Resources, Patients and Family Caregivers
Transitioning from one healthcare setting to another can be inefficient and even dangerous. Hospital discharge can result in uninformed patients and family caregivers, confusion about which medications to take and inadequate resources to meet their needs. In a first-of-its kind pilot project, Connected Health Resources and Datuit are working with a residence for low-income seniors in Maryland. Patients with multiple health issues and their family caregivers are presented with an interoperable personal health application and community resource navigation to help them live independently. A clinical pharmacist utilizes the SafeIX® Platform to access pharmacy data to reconcile medication information and generate an easy-to-understand medication schedule with instructions. Patients and families select a range of healthcare and social services from the community utilizing the Community Resource Finder. This is the first shared platform where health and social services providers, patients and family caregivers all contribute information to support the patient’s health goals.
